The Northwest Area Agency on Aging & Disability has a full-time job opening for an Options Counselor (Case Management). This position will complete in-home assessments and care plans for qualifying consumers in the counties of Crockett and Gibson. This position is located in Martin, TN. QUALIFICATIONS: BS Degree in Social Work, Psychology, Gerontology, Sociology, Counseling, Nursing or BS Degree with a minimum of two years’ experience working with older persons and/or adults with disabilities.
