MES PRE-K & KINDERGARTEN REGISTRATION TUESDAY, APRIL 9 • 8 A.M.-6 P.M. (Arrive no later than 5:30 p.m. to allow for completion of paperwork.) WEDNESDAY, APRIL 10 • 8 A.M.-3 P.M. (Arrive no later than 2:30 p.m. to allow for completion of paperwork.) Please park in the front of the elementary school and enter though the offi ce. If you are registering a child for Kindergarten, please bring them with you so that they can be tested! We are looking forward to a fantastic year! To enroll, McKenzie Elementary School must have the following documents: • Certified copy of birth certificate • Proof of Residency (i.e. utility bill with home address) • Social Security Card • Custody papers/parenting agreements if applicable •Tennessee Certificate of Immunization (forms may be obtained from your doctor or health department) • Tennessee School Entry Medical Examination completed by medical doctor (physical) The following immunizations are required and are to be documented on a Tennessee Permanent Certificate of Immunization (PH 2414): • DPT series and booster, five (5) doses • Polio series and booster, four (4) doses • MMR, two (2) doses • Hepatitis a, two (2) doses • Hepatitis b, three (3) doses • Varicella (chicken pox), two (2) doses or history of disease Students entering Pre-K must be 4 years old by August 15, 2019.
